Genre: Puzzle
Release: 1990
Platforms: Game Boy Advance (Classic NES Series, emulated in browser)

Instructions

Move the falling capsule left and right with the arrow keys, speed its descent with down, and rotate it with Z or X. Each capsule has two halves that can be different colours, so rotation is how you decide which colour lands where - and a vertical capsule that only half-matches will split, with the unmatched half dropping down on its own.

Four in a row clears, and the four can be any mix of capsule halves and viruses in any straight line, horizontal or vertical. This is the key difference from Tetris: you are not filling rows, you are building colour runs, and the viruses already in the bottle count towards them. Aim your capsules at viruses rather than at empty space.

Keep the bottle low and leave yourself a column. The game ends when the neck of the bottle is blocked, so junk that piles up in the middle is far more dangerous than junk at the edges. Setting up chains - where one clear drops halves that immediately complete another run - is the main source of big scores and the fastest way out of a bad board.

Controls

Left / Right arrows — move the capsule

Tips to Master Dr. Mario

The two games are usually shelved together and they work almost nothing alike. Tetris is about geometry: you are packing shapes to eliminate complete rows, and colour is decoration. Dr. Mario is about colour matching against fixed targets, and shape barely matters - every piece is the same two-cell capsule. The result is that Tetris rewards spatial planning while Dr. Mario rewards reading the board for the run you can close next, which is a genuinely different skill. Plenty of players are good at one and hopeless at the other.

The virus level dial

Before you start you choose a virus level from 0 to 20 and a speed setting. The virus level controls how many viruses are seeded into the bottle and how deep they sit, and it changes the game far more than the speed does. Low levels are a gentle colour-matching exercise; level 15 and above buries viruses under each other so that reaching the bottom row requires planned chains rather than opportunistic matches. Most players settle somewhere around 10 to 15 as the point where it stops being trivial and has not yet become punishing.

The half-capsule rule

The single mechanic that separates competent players from good ones is what happens to leftovers. When a clear removes one half of a capsule, the other half becomes a free-floating single cell and falls until it lands on something. Skilled play uses this deliberately: you set up a clear knowing exactly where the orphaned halves will drop, so that they complete a second run on landing. Chains built this way clear far more of the bottle than any single match can, and they are the difference between surviving level 18 and not.

A soundtrack people remember

Dr. Mario shipped with two selectable music tracks, Fever and Chill, composed by Hirokazu Tanaka. Fever in particular has outlived the game itself - it has been rearranged for later Nintendo titles and is one of the more recognisable pieces of music from the NES library. It is a small thing, but the ability to pick your track was unusual at the time and the game is much better for having a genuinely good one.
